• Clinic is FREE, but we need you to register so that we will get a good count of how many to expect.......Please Signup just once per child........Claim the appropriate item for your division/team number (ask your Manager if you don't know your team number)......When you claim your DIB item, you will be asked to supply the name of the person fulfilling the DIBS item - enter your daughter's name as you would like it to appear on the certificate that she will get at the end of the clinic.....You will also be asked to supply a phone number, please supply a phone number that will be good for emergencies on the day of the clinic. (April 27th).................CHECK IN BEGINS AT 1030am, CLINIC ENDS at 2pm
